Job SummaryTanner Health is seeking a full-time Nurse Practitioner or Physician Assistant to join the Tanner Medical Group Bridge Clinic team which is a transitional care facility designed to “bridge “the gap and support patients during the critical period between hospital discharge and a follow-up visit with a primary care provider (PCP).Job Duties & ResponsibilitiesProvide primary care services including medication management, chronic disease monitoring, coordination of care with specialists and patient education.Job Eligibility RequirementsEducation & Licensure Requires completion of an accredited Nurse Practitioner program, with a master’s degree in nursing OR master’s degree from an accredited Physician Assistant programActive GA APRN license OR GA Physician Assistant license or meet criteria for obtaining licensure.Must be board certified as a Physician Assistant. ExperiencePrior experience in adult medicine is preferred.New graduates are welcome to apply. Schedule & Location:Full-time, Monday – Friday, 8am – 5pm. Position will be located require shifts in Carrollton and Villa Rica Bridge Clinic locations.
