About us Elevation Rehab is an in-house Outpatient rehabilitation service, providing care to Durham Ridge Assisted Living and Memory Care. ```Licensed Speech Language Pathologist/Licensed Speech Therapist Job Summary``` We are seeking a skilled and compassionate Licensed Speech Language Pathologist/Licensed Speech Therapist, to join our team. As a Licensed Speech Language Pathologist/Licensed Speech Therapist, you will be responsible for evaluating, diagnosing, and treating individuals with communication and swallowing disorders. You will work closely with patients of all ages to improve their speech and language skills, enhance their communication abilities, and help them regain their independence. ```Licensed Speech Language Pathologist/Licensed Speech Therapist Responsibilities``` - Conduct thorough patient assessments to determine speech and language disorders - Develop individualized treatment plans based on patient needs and goals - Provide speech therapy services to patients with various conditions such as articulation disorders, language delays, fluency disorders, voice disorders, and swallowing difficulties - Utilize evidence-based techniques and interventions to improve communication skills - Monitor patient progress and adjust treatment plans as necessary - Collaborate with other healthcare professionals to ensure comprehensive care - Educate patients and their families on communication strategies and techniques - Maintain accurate documentation of patient evaluations, progress notes, and treatment outcomes ```Licensed Speech Language Pathologist/Licensed Speech Therapist Requirements``` - Master's degree in Speech-Language Pathology from an accredited program - Valid North Carolina state license or certification in Speech-Language Pathology - All certifications and licensing upon hire and must maintain current throughout employment. - Knowledge of anatomy, physiology, and medical terminology related to speech and swallowing disorders - Strong assessment and diagnostic skills - Excellent verbal and written communication abilities - Ability to work independently as well as part of a multidisciplinary team - Compassionate nature and strong interpersonal skills -Experience in assisted living, skilled care or memory care preferred If you are passionate about helping individuals improve their communication abilities and enhance their quality of life, we encourage you to apply for this rewarding position.
