This is a rewarding role ideal for a licensed clinician eager to make a lasting impact on the lives of underserved communities through integrated care.As a School-Based Behavioral Health Provider, You WillDeliver brief, solution-focused assessments and interventions aimed at functional restoration.Collaborate closely with primary care providers to develop and implement clear treatment plans.Provide behavioral health services, including crisis intervention and substance abuse screenings.Educate and consult with the primary care team on mental health, chemical dependency, and behavioral medicine.Support patients and families with self-management, behavior change, and harm reduction strategies.Identify referral resources and coordinate care for patients requiring specialized services.Maintain thorough documentation in Electronic Medical Records (EMR).Key ResponsibilitiesConduct comprehensive bio-psychosocial assessments addressing mental health, substance use, co-occurring disorders, and medical needs.Manage psychosocial aspects of chronic and acute diseases in collaboration with the primary care team.Be immediately available during working hours to assist with crisis interventions and psychiatric emergencies.Provide individual, group, and family interventions as needed within the school setting.Use electronic tracking tools to monitor treatment outcomes and ensure effective care coordination.Educate staff, patients, and families on mental health best practices.QualificationsMaster’s degree in Clinical Social Work, Counseling, or Psychology.Licensed in the State of Ohio (LISW, LPC, LPCC, LMFT).Three (3) or more years of experience in behavioral health, preferably in an integrated primary care setting.Strong knowledge of population-based healthcare, behavioral medicine, and evidence-based treatment models.Experience working with diverse, multicultural populations.Bilingual (Spanish/English) strongly preferred.Equal Employment Opportunity/Drug-Free WorkplaceThe HealthCare Connection is focused on creating a community that promotes dignity and respect for employees, patients and other community members.
