Registered Nurse - Center for Healthy Aging

Location: Tallahassee, FLDepartment: NursingF/T or P/T status: Full-TimeSchedule: Monday - Friday, 8:00 a.m. - 5:00 p.m.About the role:We are seeking a Registered Nurse (RN) to support patient‑centered care delivery within Capital Health Plan's Nursing department. This role provides direct patient care, health promotion, disease prevention, and patient education while working closely with providers to ensure care is coordinated, team‑based, and aligned with community needs.The Registered Nurse identifies patient care needs through in‑person, telephone, or electronic interactions, documents care in the Electronic Health Record (EHR), develops and implements plans of care, and performs patient care services in accordance with the Nurse Practice Act and Capital Health Plan's policies and procedures. This role also supports proactive care by identifying care gaps and facilitating appropriate follow‑up.We're looking for someone who has:Associate's (ASN) or Bachelor's degree (BSN) in Nursing from an accredited college or universityActive Registered Nurse (RN) license in the State of FloridaBasic Life Support (BLS) certificationMinimum of two years of related nursing experience preferredAbility to assess patient needs and provide care through in-person, phone, or electronic modalitiesAbility to document patient information accurately in an Electronic Health RecordStrong knowledge of infection control practices, medication administration, and patient safety standardsAbility to communicate effectively with patients, providers, and interdisciplinary care teamsHighly preferred candidates also have:Experience working in outpatient, primary care, or ambulatory settingsExperience identifying and addressing gaps in care using clinical tools or registriesExperience developing and implementing care plans independently and collaborativelyStrong proficiency with clinical systems and healthcare technologyKnowledge of CHP policies, workflows, or population health conceptsAbout Capital Health Plan (CHP):CHP is a locally based, not‑for‑profit health maintenance organization serving the Tallahassee region for more than four decades.
