We are committed to providing our clients with the best possible and most compassionate care!Job SummaryAs a Coordinator at Elite Home Health Care, you will play a pivotal role in ensuring the smooth and efficient coordination of home care services for our patients. You will be responsible for managing an assigned caseload, strategically coordinating the placement of services between clients and caregivers and maintaining successful caseload management to ensure timely and appropriate care delivery. Additionally, you will collaborate with team members to address staffing issues, update patient and caregiver records accurately and promptly, and assist with on-call duties as scheduled. Your strong leadership, problem-solving, and communication skills will be essential in providing high-quality care and support to our patients and their families.Essential Duties and Responsibilities: Responsible for the assigned caseload, ensuring that patients receive the best possible home care servicesStrategically coordinate placement of services between clients and caregivers using HHA eXchangeMaintain successful caseload management and ensure timely and appropriate care for patientsAttend departmental training and meetings to stay updated on policies, procedures, and best practicesAssist the team lead with coverage and collaborate with team members to address staffing issuesDiscuss schedule, availability, attendance, and other important issues with department staff members openly and constructivelyUpdate patient and caregiver records in HHA eXchange accurately and promptlyVerify caregivers’ attendance daily, prioritize staffing cases for no-shows or call-outs based on patient needsReceive and handle calls, emails, or faxes from contractors and clients requesting information or updates on reports; discuss patient status with nurse care managersReceive complaints from caregivers about working conditions or policies and collaborate with management to resolve issues in the employee's best interestsAssist with on-call duties as scheduledQualifications and Skills:General experience in healthcare or a related area preferredProficient in Microsoft Office Suite, HHA Exchange, and internet navigationStrong leadership and problem-solving skillsExcellent organizational abilities and attention to detailStrong oral and written communication skills, capable of effectively communicating with employees, patients, and their familiesAbility to define problems and goals, collect data, establish facts, take action, and address issuesAbility to multitask in a fast-paced environmentFlexible schedule to accommodate the needs of the department and patientsPhysical Demands:Must remain in stationary position for long periods of time at desk or computerRequires occasionally standing, sitting, walking; using hands to finger, handle, or feel objects, tools or controls; reaching with hands and arms; climbing stairs; hearing; talkingWork Environment: On-siteBenefits:Medical InsurancePTO & Sick TimeReferral ProgramPay Range: The hiring range for this position is $25.00-$27.00/hour.
