This full time position will split duties between our Peru and Logansport clinics. You will be crucial in providing integrated behavioral health services within the Primary Care Behavioral Health model. The Behavioral Health Professional is an essential member of our integrated care team and is vital in delivering evidence-based and high-quality care to the individuals and families we serve in our community.Meet our Peru Team: https://indianahealthonline.org/locations/peru/Meet our Logansport Team: https://indianahealthonline.org/locations/logansport/Role Highlights:100% outpatient4-day work week optionDedicated administrative time built into your scheduleNo weekends, evenings, off on major holidays, and generous PTOAdministrative supportMission-driven hybrid flexibilityDedicated case managerOngoing professional development trainingChallenging and rewarding work environmentIHC's robust benefits and compensation package includes:$5,000.00 retention bonus paid after one year$2,500.00 paid for continuing education, including paid licensing fees and tuition reimbursementPaid malpracticeDay 1 Insurance benefits eligibilityEmployer-paid Group Life, Short-term disability, and Long-term disability coverages and HSA employer contributions403(b) Retirement Plan matching at one year of employmentGenerous Paid Time Off and Floating HolidaysFlexible Leave of Absence programsPersonify Health Wellness program with paid incentives for participationSupportLinc Employee Assistance Program with 24/7 access to therapy consultation servicesLicensed Behavioral Health Professional Job Responsibilities:Provide comprehensive assessment and diagnosis of behavioral health patientsAssists providers in recognizing and treating mental, psychosocial, and substance abuse disordersProvide effective treatment planning and assist patients in achieving goalsEvaluate crises and apply appropriate interventions and referralsProvide assessment, consultation, and brief intervention for psychological/psychiatric disordersMaintain an active presence and communicate with Primary Care Providers (PCPs) during clinic hoursAvailable for same-day and scheduled initial interventions with patientsPerforms brief, limited follow-up visits for selected patientsActively participate in meetings that support IHC's integrated healthcare model to provide comprehensive care for patientsAssist in the detection of "at risk" patients and the development of plans to prevent further psychological or physical deteriorationAssist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease, including substance useTeach patients, families, and staff care, prevention, and treatment enhancement techniquesMonitor the site's behavioral health program, identifying problems related to patient service and make recommendations for improvementDesired skillset:The ideal candidate will have excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditionsComfortable with the pace of primary care, working with an interdisciplinary teamAbility to design and implement clinical pathways and protocols for treatment of chronic conditionsAbility to work through brief patient contacts and make quick and accurate clinical assessmentsStrong communication skillsCultural awareness and sensitivityGood knowledge of psychopharmacologyRequirementsMaster's degree in social work from an accredited university/collegeLCSW, LMFT, or LCMHC licensure in the state of IndianaBasic Life Support (BLS) certification (AED included)Active and clean Indiana driver's license, including state-mandated auto insuranceAt least 1 year of experience, 3 years or more preferredEqual Employment Opportunity StatementWe are an equal-opportunity employer.
