Responsible to oversee and coordinate the homeless outreach activities at street level, contracting with Medicare health plans and other agencies to ensure that individuals experiencing homelessness receive the appropriate medical care, support, and resources. In this role, the Homeless Services Program Manager will spearhead the successful implementation of performance measures, meeting program needs and developing efficient data collection. Additionally, collaborate with multidisciplinary teams to ensure coordinated and effective client care while instilling our shared vision for street medicine practice.Responsibilities:Oversees program requirements and establishes workflows for healthcare delivery of high-risk populations such as individuals experiencing homelessness.Establish and maintain partnerships with government agencies, shelters, and other organizations to enhance the program outreach efforts.Collaborates with medical professionals, social workers, and outreach teams to provide comprehensive healthcare services on the streets.Oversees and supports the non-clinical team that conducts care management activities such as: enrollment, individual care planning, housing navigation, and other documentation.Monitor direct reports caseloads and overall care coordination and housing navigation across multiple systems/platforms to ensure targeted goals and desired outcomes are met.Demonstrates strong interpersonal, critical thinking and analytical skills through regular communication with Care Management team and with internal/external stakeholders.Maintains program compliance and promotes a culture of continuous improvement in client care.Facilitates meetings to provide updates on program needs and develops strategies for program quality assurance and improvement.Responsible for managing other organizational programs such as Community and Population Health, Environmental and Hygiene Health, Maternal and Children Health, Refugees and Immigrant Health, Institutional Health, Health Education, Health Promotion and Health Prevention.Develop, evaluate, and monitor public health program budgets and ensure proper administrative and financial controls.Supervise staff under the Homeless Services' Program.Build and maintain professional rapport with a diverse stakeholder including, but not limited to, Governmental Agencies, public and private agencies, community organizations and professional groups.Qualifications/Requirements:Bachelor's degree in Public Health, Health Care Administration, Healthcare Policy, Social Work or related field plus (3) years of professional experience in street medicine, homeless care management, or other related field.Experience with street outreach is preferred.Understanding and/or familiarity with the principles of project and program management.Familiarity with laws and regulations governing county, state, and federal-funded programs and the non-profit sector.Experience in working in health care programs, public health, social services, homeless services, or working with vulnerable populations.Ability to work with all levels of staff, within and external to the organization to achieve program goals.2 years of community health experienceWork cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational and program effectiveness.English/Spanish bilingual is requiredPossess a valid California driver's license at time of appointment and maintain.
