This role also includes facilitating communication between residents, families, and staff, coordinating discharge planning, and ensuring compliance with regulationsKey Responsibilities:Assessing needs, plan and manage all aspects of our comprehensive activities program, working with department resources, team members, and volunteersGetting to know each resident individually to better understand needs and varying interests and coordinate activities accordinglyCompleting screens and orientating residents and families to Oak Park PlaceHelping residents and families adjust to the centerAssisting families and residents with financial questions and referrals RequirementsSocial Worker Qualifications:Associate degree preferredPrevious experience in a supervisory rolePrevious experience working with seniors is preferredStrong organizational and planning skills requiredAbility to work independently is a must
