Licensed Master Social Worker Location: Bethel, AK Agency: Acruex Health Pay: $1,408 to $1,536 per week Shift Information: Days - 4 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Pay Rate: $44.00 - $48.00/Hourly. Summary: The Acute Care Social Worker – Discharge Planning coordinates safe and timely transitions of care for hospitalized patients. This role focuses on psychosocial assessments, discharge coordination, resource linkage, and collaboration with interdisciplinary teams to reduce readmissions and ensure continuity of care. Responsibilities: ⁠Conduct psychosocial assessments for hospitalized patients. Identify discharge needs including home health, skilled nursing, rehab, hospice, DME, and community resources. Coordinate with physicians, nurses, case managers, and therapy staff. Arrange placement in post-acute facilities. Assist with insurance authorization and payer requirements. Provide crisis intervention and emotional support. Requirements Requirements: MSW (Master of Social Work) from an accredited school of Social Work One (1) year experience in hospital or health-care setting.
