This leadership role oversees chronic care management, infection control, and occupational health processes while collaborating with clinical leadership on best practices and patient care excellence.Key Responsibilities• Staff Leadership & ManagementSupervise nursing managers and chronic care management programsEnsure adequate RN, LPN, and MA staffing and competencyDevelop KPIs for staffing and performance metrics• Clinical OperationsOversee nursing services, policies, and proceduresManage infection control and occupational health protocolsCoordinate vaccine programs and regulatory compliance• Quality & ComplianceParticipate in regulatory survey readiness (Joint Commission, NCQA, DPH)Review patient satisfaction surveys and incident reportsSupport clinical quality initiatives and safety programs• Education & DevelopmentCreate onboarding and continuing education curriculaFoster professional development and staff retentionImplement nursing innovations and care delivery modelsRequired Qualifications• Education: Master's Degree preferred• Experience: 5+ years RN experience with 3+ years supervisory experience• Licensing: Current Massachusetts RN license required• Specialization: 3+ years primary care nursing experience• Management: 3+ years nurse management experiencePreferred Qualifications• Bilingual capabilities (Spanish, Portuguese, or Haitian Creole)• Primary care triage experience• Patient Centered Medical Home model knowledge• EMR and Microsoft Office proficiency• Experience with diverse patient populations• Healthcare system management experienceWhat We OfferCompetitive salary-Salary Range:$105,000 - $134,000 annuallycomprehensive benefits, and the opportunity to make a meaningful impact in primary care healthcare serving diverse communities.
