This role supports patients with acute kidney injury, chronic kidney disease, end-stage renal disease, electrolyte disorders, and dialysis-related needs. The APP evaluates, diagnoses, and manages complex renal conditions, develops treatment plans, coordinates care, and provides patient and family education. This role requires strong clinical judgment, teamwork, and a commitment to evidence-based care and patient safety.Experience • Hospital experience preferred, but not required• Nephrology experience is a plus• 3 years of APP experience preferred, but we are willing to train the right candidate with a strong clinical foundation, the right mindset, and a commitment to learning nephrology careMinimum Qualifications • Licensed to practice in North Carolina as a Nurse Practitioner or Physician Assistant, in good standing with applicable licensing and certifying bodies• Graduate of an accredited NP or PA program• Current national certification applicable to role (ANCC/AANP for NP, NCCPA for PA)• Current BLS and ACLS certification, and ability to maintain both• Clear criminal background history• Ongoing continuing education and license maintenance• Valid driver’s license and reliable transportation for support across office, hospital, and dialysis locations as needed• Working knowledge of Microsoft Office and familiarity with Electronic Health RecordsPreferred Qualifications • Experience in inpatient nephrology, hospital medicine, critical care, or hospitalist practice• Experience managing dialysis patients in the hospital setting• Familiarity with hospital-based EHR systems• Strong communication skills and patient education skillsKey Responsibilities • Conduct patient assessments, including history, physical examination, and review of diagnostic testing related to nephrology care• Develop and implement individualized care plans in collaboration with nephrologists and the healthcare team• Manage inpatient dialysis-related care and monitor for complications or changes in clinical status• Order and interpret laboratory and diagnostic studies within scope and protocol• Provide education and counseling to patients and families• Participate in multidisciplinary rounds, case conferences, and quality improvement activities• Document patient encounters, treatment plans, and progress notes accurately and timely in the electronic medical record• Seek consultation from collaborating physicians when cases fall outside established protocols or scopeIn-Person Requirement This position is designated as an on-site, patient-facing role. In-person presence is an essential function due to the need for real-time collaboration with providers, direct interaction with patients and caregivers, and secure handling of health data in compliance with HIPAA. All essential functions listed below are expected to be performed on-site.
