In this role, along with Admission / Accessments / Reacessments, you will primarily create and manage a care plan for individual patients. You are a Registered Nurse (RN) with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and can observe and communicate effectively in your patient's best interest.
