Per Diem Nurse Clinician
Kennebec Pharmacy & Home Care has an opening for a per diem/part-time IV Nurse Clinician at our Augusta location.
POSITION SUMMARY:
The Nurse Clinician provides and coordinates patient care to infusion patients at home or in an alternative setting.
ESSENTIAL FUNCTIONS:
Performs patient care in accordance with State laws, and Kennebec Pharmacy & Home Care policies and procedures.
Completes all appropriate documentation accurately and in a timely manner.
Performs clinical duties according to physician orders.
Communicates appropriately with the pharmacy manager, physician, and other health care members to ensure quality of patient care.
Participates in “on-call” rotation and patient-related travel as necessary.
Participates in scheduled patient conferences.
Assists in the orientation of other nurse clinicians.
Educates patients, caregivers, referrals, home health agencies, etc., as appropriate.
Patient visits may include:
Physical assessment
Psycho-social support
Specific nursing procedure
Administration of medication
Therapeutic monitoring
Development and revision of Plan of Care
Patient/caregiver education
Coordination of related services
Coordinates, participates in, and reports the activities and outcome results of the Improving Organization Performance program.
OTHER RESPONSIBILITIES:
Takes responsibility for self-protection while working in hazardous situations, and practices universal precautions.
Always maintains professional appearance and attitude.
Maintains patient confidentiality and rights.
Clearly understands the purpose of and participates in the Improvement Organization Performance program.
Performs other work-related duties as assigned by supervisor.
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
Accreditation from a school of nursing.
Current State of Maine RN License.
Current CPR certification.
Minimum of two years in an acute care setting.
Previous home care experience preferred, or at least one year of nursing experience within the past three years.
Strong knowledge of I.V. Therapy preferred.
Effective communication, teaching, and presenting skills.
SUPERVISORY RESPONSIBILITIES:
None assigned.
PHYSICAL AND ENVIRONMENTAL REQUIREMENTS:
Standard office setting with minimal noise level. In-state travel to patients’ homes required.
