As a Registered Nurse in this role, you have the time and support to provide the best 1:1 care to our clients right in their own home environments. Registered Nurse / RN Case Manager Overview: Provide quality nursing services to private-pay clients throughout the Twin Cities Metro Area Deliver impeccable customer service and care to our clients in their home environment Perform ongoing client assessments, skilled nursing visits, and caregiver training and supervision Interest in process improvement and caregiver training/mentoring On-Call rotation Registered Nurse / RN Case Manager Qualifications: Licensed as a Registered Nurse / RN in the state of Minnesota Must have a reliable vehicle with current insurance and a valid Driver's License Exceptional nursing, assessment, and customer service skills Excellent at planning, organizing, communicating, and thinking critically Prior Nursing / RN experience in a Home Health setting is ideal About Legacy Home Care: Legacy Home Care is a top-rated Twin Cities' private pay home care agency.
