Hospice RN Case Manager

If you are passionate about delivering high-quality, patient-centered care and want to be part of an organization that treats both patients and employees like family, we would love to welcome you to our team.Key ResponsibilitiesPerform comprehensive patient assessments and ongoing evaluationsDevelop, implement, and revise individualized care plansAdminister medications, injections, and IV therapies as prescribedProvide wound care and monitor healing progressMonitor and manage chronic conditions (e.g., diabetes, heart failure, COPD)Educate patients and family members on disease management, medications, and safetyDocument all patient care accurately in electronic health records (EHR)Communicate changes in patient condition to physicians and care teamsCoordinate care with physical therapists, occupational therapists, social workers, and other healthcare professionalsEnsure compliance with agency policies, state regulations, and federal guidelinesParticipate in case conferences and quality improvement activities Required QualificationsActive Registered Nurse (RN) license in the state of practiceGraduate of an accredited nursing programCurrent BLS/CPR certificationMinimum of 1–2 years of clinical nursing experience (home health experience preferred)Valid driver's license and reliable transportationAbility to work independently and manage time effectively Preferred QualificationsExperience in home health, hospice, geriatrics, or medical-surgical nursingStrong wound care and IV therapy skillsKnowledge of Medicare/Medicaid regulationsOASIS documentation experience Skills & CompetenciesStrong clinical assessment skillsCritical thinking and sound clinical judgmentExcellent communication and patient education skillsCompassion and empathyOrganizational and time management skillsAbility to adapt to various home environments Work EnvironmentPatient homes in assigned geographic areaFrequent local travel requiredFlexible scheduling may include weekends or on-call rotationsCompany DescriptionMichigan Community VNA strives to compensate their employees in the high-end of market and all compensation is negotiable.Michigan Community VNA is searching for Home Health RN's who are looking for a flexible work schedule, great benefits, compensation and a great team.
