Responsibilities Evaluate and treat patients in a post-acute/SNF setting, developing evidence-based PT treatment plans. Deliver skilled PT interventions targeting mobility, strength, balance, pain management, and functional independence. Document evaluations, daily notes, progress notes, and discharge summaries within required timeframes. Collaborate with OT, SLP, nursing, and the interdisciplinary team to coordinate resident-centred care. Educate patients and families on home exercise programmes, fall prevention, and safe discharge planning. Meet facility productivity standards while maintaining the highest standard of clinical care. Requirements Current, unencumbered Physical Therapist licence in Pennsylvania required before start. Graduate of an accredited Physical Therapy programme (DPT preferred). Minimum 1 year of SNF or post-acute rehabilitation experience preferred. Current BLS/CPR certification. Strong documentation skills and ability to meet productivity targets. Apply now:
