Transition Specialist

This specialist works directly with customers to meet their needs and get back into the community. This will include in home visits, shopping, contacting community resources, phone calls, data entry, document review, and other duties such as:Providing assistance in identifying and securing a safehome in the communityProviding support in obtaining furnishings, household items, DME and items necessary for transitioning to live independentlyAssistance with finding community resources and supports for HCBS servicesCreate a budget with customer of deposits, items needed, monthly rent for a successful move back into the communityProviding follow up after move is complete: three days after move,14 days, and 30 days after moveCoordinate move date: landlord, utilities, HCBS services, deposits, household items, medications, follow up doctors appointmentProvide training and materials for customers to understand their rights as they leave the nursing home and back into the community ```{=html}```Must have strong oral and written skillsStrong customer service skills; able to work with a diverse group of peopleWork independently and also as part of a teamPosses a strong belief that all people have the same rights and can live independentlyAbility to teach others about their rights and responsibilitiesWillingness to explore resources that are needed for consumer to be independentMust have strong oral and written community skillsMust meet the eligibility requirements of the auto insurance company providing coverage for OCCK vehiclesFlexible schedule to meet customer needs, home visits and travel requiredValid Kansas Drives License or have access to transportation
