This role focuses on improving patient mobility, function, safety, and independence while ensuring compliance with agency policies and regulatory standards.Essential ResponsibilitiesEvaluate patients and interpret assessment findings to determine functional limitations, treatment needs, and prognosisDevelop individualized treatment goals and establish appropriate physical therapy plans of careProvide skilled physical therapy treatment in accordance with the established plan of careInitiate and participate in discharge planning, modifying treatment goals as patient progress warrantsDelegate appropriate components of the treatment plan to supportive personnel in accordance with regulations and agency policyCoordinate patient care with other members of the interdisciplinary healthcare teamEducate patients and family members on treatment plans, safety, and home exercise programsRegularly reassess patients to determine effectiveness of treatment and need for plan modificationsAssess patient needs for adaptive equipment related to Activities of Daily Living (ADLs) and coordinate with approved equipment vendors as neededReassess adaptive equipment for effectiveness and recommend modifications when appropriateDischarge patients from physical therapy services when maximum benefit has been achievedSupervise Home Health Aides in accordance with agency policy and regulatory requirementsMaintain accurate, timely, and compliant clinical documentation in accordance with agency policies and payer requirementsDemonstrate ongoing professional development and adherence to standards of professional practiceRequired QualificationsGraduate of an accredited Physical Therapy programCurrent Physical Therapist license in the state of practiceMinimum of one (1) year of experience as a Physical Therapist; home health experience preferredKnowledge of home health regulations and documentation standardsStrong clinical assessment, organizational, and time-management skillsExcellent verbal and written communication skillsAbility to work independently and manage a flexible scheduleAbility to lawfully work in the United StatesPhysical RequirementsAbility to travel between patient homesAbility to lift, transfer, and position patients as neededAbility to stand, walk, bend, and reach during patient care activitiesAbility to communicate effectively with patients, families, and healthcare professionalsCompensationPer-visit reimbursementMileage reimbursementWe are an Equal Opportunity Employer and do not discriminate against any employee or applicant for employment based on race, color, religion, sex, sexual orientation, gender identity, national origin, age, disability, genetic information, veteran status, or any other characteristic protected by applicable federal, state, or local laws. xaygatp We are committed to creating a diverse and inclusive workplace for all employees.
