Hourly Rate: $60-65/hr.Contract: 6+ monthsSchedule Notes: 0700 AM – 1930 PM, including weekends (10 hours * 4 days schedule)Location: The Woodlands, TX 77384About the CompanyTo promote pharmaceutical care by managing the medication process, identifying and preventing potential drug-related problems, and resolving actual drug-related problems. Additionally, to perform the duties of a registered pharmacist as defined by Texas Children's Hospital. All responsibilities will be performed in a manner developmentally appropriate for the ages of patients served (0-adult).About the RoleThe pharmacist in this role shall be a licensed practitioner who holds a valid, unrestricted license to conduct clinical activities.ResponsibilitiesEnsures the appropriateness of all medication doses in reference to the patient’s diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by lack of medication incidents.Accurately enters medication orders in computer as evidenced by lack of medication incidents.Ensures the medications the patient receives are correct as documented by lack of dispensing errors file.Ensures medication orders are processed and doses are delivered within established guidelines as evidenced by peer review or by the lack of medication incident/customer complaint.Follows aseptic technique and infection control in the preparation of sterile products. Insists technicians and other pharmacists practice aseptic technique and infection control. Completes annual re-certification and scores at least 90% on the aseptic technique/infection control skills and written exam as documented by a qualified observer.Ensures medications meet the highest standards.Participates in the education of patients, caregivers, and the community.Supports patient safety systems.Checks all available means of communication including e-mail, mailbox, and voicemail every shift.Ensures the smooth running of the pharmacy and acts as a role model for technicians and other pharmacists as evidenced by lack of complaints, medication incident reports, and peer review.Maintains investigational drug records according to departmental policy.Contribute to the medication management process.Conduct discharge medication review (difficult to obtain, prescription accuracy) and counseling, ensuring that patients/caregivers understand their medication therapies upon discharge.Participates in study initiation, maintenance and closure.QualificationsRequired - Bachelor's Degree Pharmacy Or Ph.D. Pharm.
