DescriptionABOUT THE ROLEActiva Home Health is expanding its Hospital at Home program, delivering hospital-level care to patients in their homes as an alternative to inpatient hospitalization. Patients remain formally admitted to a hospital partner while receiving coordinated in-home care supported by physician oversight and continuous clinical monitoring. POSITION SUMMARYProvide hospital-level skilled nursing care to patients enrolled in a Hospital at Home program. RNs deliver high-acuity care in the home while completing timely, accurate clinical documentation in Kinnser (WellSky) .
