DescriptionABOUT THE ROLEActiva Home Health is expanding its Hospital at Home program, delivering hospital-level care to patients in their homes as an alternative to inpatient hospitalization. Patients remain formally admitted to a hospital partner while receiving coordinated in-home care supported by physician oversight and continuous clinical monitoring.POSITION SUMMARYProvide hospital-level skilled nursing care to patients enrolled in a Hospital at Home program.
