We are excited to launch a new Milwaukee County team and are seeking Care Coordinators to support children with developmental, physical, or severe emotional disabilities through the Children's Long Term Support (CLTS) Waiver Program . Role Overview As a CLTS Care Coordinator, you will serve as the primary point of contact for families, helping them access services that keep children safe and thriving in their home and community. This role involves assessment, service planning, coordination, and ongoing support. Key Responsibilities Conduct functional assessments and reassessments using approved toolsDevelop individualized service plans with families and providersAuthorize and coordinate services in alignment with the planFacilitate family-centered team meetingsMaintain accurate documentation and client recordsCollaborate with service teams and supervisors to determine interventionsTestify at legal proceedings when requiredParticipate in staff development, supervision, and trainingDirect Service Expectations Promote client independence and growthProvide trauma-informed, family-centered servicesUtilize evidence-based practices supported by LSS and fundersComplete time reporting accurately and on scheduleQualifications Bachelor's degree in a health or human services field (examples: Social Work, Psychology, Counseling, Special Education, Occupational Therapy, Sociology, Criminal Justice, etc.)Minimum of 1 year of experience working with children in the target populationBilingual fluency in Spanish is preferred but not required Valid driver's license, reliable transportation, and satisfactory driving recordStrong communication, documentation, and organizational skillsAbility to use electronic health records and computer applicationsPerks & Benefits Medical, Dental, and Vision InsuranceFlexible Spending Accounts (Health & Dependent Care)Mileage reimbursementPaid Time Off + 10 Paid Holidays403(b) retirement plan with contribution optionsEmployee Assistance ProgramService awards and recognitionRemote work option: 2 days per week from homeWork Environment Community-based role with daily travel requiredOccasional overnight travel may be necessaryModerate noise level; exposure to outdoor conditions during travelAbility to respond appropriately in crisis situationsLSS is an Equal Opportunity Employer (EOE) Qualifications Education Bachelor Degree of Hum.
