Role Overview: As a Care Manager, you'll play a crucial role in transforming the lives of our senior clients through compassionate care. You'll be responsible for developing and managing individualized care plans, training and coordinating caregivers, and ensuring the highest standards of care. Your leadership and organizational skills will help us provide top-notch support and advocate for the well-being and dignity of each senior we serve. Key Responsibilities: Develop and Manage Care Plans: Create and oversee individualized care plans tailored to each client's needs. Train and Coordinate Caregivers: Ensure caregivers are well-trained and matched with clients for optimal care. Schedule and Conduct Consultations: Organize and lead client and family consultations to address needs and expectations. Ensure Quality Care: Monitor care delivery to ensure compliance with standards and provide exceptional service. Support Caregivers and Families: Offer guidance and support to both caregivers and families to enhance the care experience. What We're Looking For: Passion for Eldercare: A strong commitment to improving the quality of life for seniors. Experience in Senior Care/Management: Proven background in caregiving or healthcare management. Organizational and Leadership Skills: Ability to lead, communicate effectively, and adapt to changing priorities. Additional Experience: Previous caregiving experience and healthcare management education are a plus. Benefits: Competitive Salary: $40,000 - $45,000 per year (based on experience).
