The SLP will develop and implement appropriate plans of care in accordance with all regulatory and clinical practice standards.Key Responsibilities: Utilize standardized assessments and evidence-based practices to support clinical interventions. Assess patient progress and adjust treatments as needed, documenting changes in compliance with regulatory and clinical requirements. Evaluate treatment outcomes for effectiveness and modify services as necessary. Provide consultation and counseling to patients and their families, caregivers, and other service providers regarding speech disorders. Communicate effectively with supervisors and other healthcare team members about patient progress, barriers, and treatment plans. Prepare comprehensive discharge summaries that comply with regulatory and clinical standards. Document all required information in the electronic documentation system, maintaining patient records according to company policies. Record treatment minutes accurately as per all regulatory and clinical guidelines. Participate in required meetings, including patient care conferences and rehabilitation meetings. Engage in facility and company-required trainings, in-services, and conferences. Obtain any necessary Durable Medical Equipment (DME) or supplies as per regulatory and clinical guidelines. Supervise and support Clinical Fellows (CFY) and Student Speech-Language Pathologists as needed. Adhere to ASHA and state practice acts, including the codes of conduct and ethics during professional practice.Qualifications: Minimum of 1 year of experience as a Speech and Language Pathologist. Experience in Long Term Care or Rehabilitation settings. Active Oklahoma State License. CCC certification from ASHA.
