Salary:CNA/LPN 1:1 Direct Support PositionPosition Title: CNA or LPN 1:1 In-Home Support StaffLocation: Group Home Setting with one personSchedule: Full-Time and Part-Time Opportunities Available | 24/7 CoverageReports To: Designated Coordinator/Designated ManagerPosition SummaryWe are seeking a compassionate, dependable, and patient-focused CNA or LPN to provide dedicated 1:1 support services for an individual living independently in their own home. This position plays a vital role in supporting the person served with daily health and personal care needs while encouraging independence, comfort, safety, and community involvement.The ideal candidate will enjoy building meaningful relationships and assisting someone who loves coloring, shopping trips, and relaxing outdoors to enjoy the weather.Essential ResponsibilitiesPersonal Care & Daily Living SupportAssist with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility supportMaintain a clean, safe, and organized home environmentMedical & Health SupportAdminister medications according to physician orders and company policiesProvide tube feeding care and monitoringObserve and document changes in health status, behaviors, or medical concernsMonitor for signs of illness, discomfort, or medical emergenciesCommunicate effectively with healthcare providers, guardians, nursing staff, and supervisorsCommunity & Recreational SupportAccompany and support the person served during shopping outings and community activitiesEncourage participation in preferred hobbies and activities such as coloring and spending time outdoorsPromote independence, choice-making, and quality of lifeDocumentation & ComplianceComplete accurate daily documentation and chartingFollow individualized support plans, medical protocols, and MN 245D policies/proceduresMaintain confidentiality and professional boundaries at all timesAttend required trainings and staff meetingsQualificationsCurrent CNA certification or active LPN licenseValid drivers license and acceptable driving recordAbility to pass background study requirementsExperience working with individuals requiring high levels of medical and personal care preferredExperience with tube feeding and medication administration preferredCompassionate, reliable, patient, and self-motivatedPhysical RequirementsAbility to stand, walk, bend, and assist throughout shiftsAbility to respond appropriately during emergenciesWhy Join Our Team?This position offers the opportunity to make a direct and meaningful impact in someones daily life by helping them remain safely and comfortably in their own home while supporting the activities they enjoy most.
