Renal Social Worker (LCSW)

DialySuites is currently looking for a passionate dialysis Social Worker to be an empowering advocate for our patients. The ideal candidate will have a passion and ability to assess, educate, and collaborate with patients and team on care planning. Responsibilities include developing and maintaining a professional therapeutic relationship with patients, orienting patient and family with other team members, exploring family and community resources, participating in facility care planning meetings, providing ongoing casework, and understanding, supporting, interpreting and implementing the philosophy and policies of the facility. Position will be full-time with an expected 30 hours per week, with flexible hours. Dialysis experience preferred, (MSW with current/active FL LCSW).
