RN - Admit Nurse & Marketer

Our work environment includes: Modern Office Setting Efficient Team Work Lively Atmosphere Growth Opportunities Knowledgeable and Dedicated Staff Frequent Employee Recognitions Responsibilities: Assess potential Hospice or Home Health patients in a timely manner Educate patient, family, and caregivers on Hospice and Home Health services Conduct patient assessments to determine their needs and develop individualized care plans Include the patient and caregiver in the development of the care plan Collaborate with healthcare professionals to coordinate diagnostic evaluations and treatment plans Provide direction to LPN to ensure patients needs are met Utilize knowledge of physiology and medical conditions to provide appropriate care and interventions Perform educational visits to potential patients, providers, or facilities as requested Participate in IDT and assist in Home Health to Hospice conversions Conduct utilization management activities to ensure appropriate use of healthcare resources Provide education and support to patients and their families regarding their healthcare options and treatment plans Coordinate delivery of needing supplies, equipments, and medications in a timely manner Provide education to community partners on hospice and home health and assist in business development Perform Case Management responsibilities including but not limited to updating plan of cares, performing recertifications, and supervisory visits. Skills: In-depth knowledge of physiology and medical conditions, allowing for accurate assessment and intervention for both Hospice and Home Health Proficient in conducting patient assessments and utilizing diagnostic evaluations to inform care plans Excellent patient service skills, with the ability to provide compassionate and comprehensive care Experience in hospice and good understanding of qualifications for hospice.
