The Home Infusion Pharmacist is responsible for the safe and accurate preparation, dispensing, and monitoring of intravenous (IV) medications administered in a patient's home or alternate care setting. This role ensures compliance with all regulatory standards while collaborating with healthcare providers to optimize patient outcomes in a home infusion environment.Key Responsibilities:Review and verify physician orders for home infusion therapies, including antibiotics, total parenteral nutrition (TPN), chemotherapy, and specialty biologicsCompound and supervise the preparation of sterile IV medications in accordance with USP and standardsEnsure accurate dispensing and labeling of medications and supplies for home deliveryProvide clinical oversight, including dosing adjustments, therapeutic monitoring, and patient-specific care planningCollaborate with physicians, nurses, and case managers to coordinate patient careCounsel patients and caregivers on medication administration, storage, side effects, and safetyMonitor patient outcomes and laboratory results; intervene as necessaryMaintain compliance with state and federal regulations, accreditation standards, and company policiesOversee pharmacy technicians and support staff in compounding and workflow processesParticipate in on-call rotation for clinical support and urgent patient needsQualifications:Doctor of Pharmacy (PharmD) or Bachelor of Science in PharmacyActive pharmacist license in the applicable stateExperience in sterile compounding and home infusion or hospital pharmacy preferredKnowledge of USP , USP , and aseptic technique standardsStrong clinical knowledge of IV therapies and disease statesExcellent communication and organizational skillsPreferred Qualifications:Board certification (e.g., BCPS)Experience with home infusion pharmacy servicesFamiliarity with electronic medical records and pharmacy management systemsWork Environment:Pharmacy cleanroom and office settingMay include occasional patient interaction (in-person or virtual)On-call or weekend rotations may be requiredPhysical Requirements:Ability to stand for extended periodsManual dexterity for sterile compoundingAbility to lift and transport medication supplies (up to ~25 lbs)Key Competencies:Attention to detail and accuracyClinical decision-makingPatient-centered careRegulatory complianceTeam collaboration
