In these roles, you will provide personalized, patient-centered therapy services in patients' homes, helping them improve mobility, reduce pain, restore function, and regain independence following illness, injury, or surgery.Service LocationsWe are currently hiring for positions in the following areas:Nashville, TNMurfreesboro, TNRobertson CountyCheatham CountySumner CountyKey ResponsibilitiesFor Physical Therapists (PTs):Evaluate patients' physical conditions and develop individualized treatment plansImplement therapeutic exercises and interventions to improve strength, balance, and mobilityMonitor patient progress and adjust treatment plans as neededEducate patients and caregivers on exercises, safety, and injury preventionDocument evaluations, treatments, and outcomes accuratelyCollaborate with physicians, nurses, PTAs, and other healthcare professionalsEnsure compliance with all healthcare regulations and company policiesFor Physical Therapist Assistants (PTAs):Implement treatment plans developed by the supervising Physical TherapistAssist patients with therapeutic exercises and functional mobilityMonitor patient progress and report changes to the supervising PTEducate patients and caregivers on exercises, safety, and proper techniquesDocument treatments and patient responses accurately and in a timely mannerMaintain a safe, supportive environment during therapy sessionsComply with all healthcare regulations and company policiesQualificationsPT Requirements:* Active Physical Therapist (PT) license in the state of employment* Minimum of 1-2 years of experience (home health experience preferred)PTA Requirements:* Active Physical Therapist Assistant (PTA) license in the state of employment* Previous experience in home health or outpatient settings preferredFor Both Roles:Strong clinical and assessment skillsExcellent communication and patient education abilitiesAbility to work independently and manage time effectivelyCompassionate, patient-focused approachCurrent CPR certificationWhat We OfferCompetitive payFlexible schedulingSupportive, team-oriented work environmentOpportunities for professional growth and advancementJoin All Heart Home Care and make a meaningful difference by helping patients regain strength, mobility, and independence through high-quality, compassionate in-home therapy.Ready to Get Started?Email ELITE to TLayne@allheartfamily.comInclude your name, resume, and a short description of your care background.
