Together, we transform lives. ▶️ See how we create comebacks.   What You’ll Do Evaluations & Assessments: Conduct comprehensive evaluations of consumers as requested by prescribing physicians, including assessments of strength, balance, endurance, coordination, joint range of motion, muscle flexibility, gait, and movement patterns. Treatment & Intervention: Deliver individualized physical therapy services, including designing, instructing, and monitoring home exercise programs for clients and direct care staff. Prepare clinical treatment plans and progress notes, and arrange for any necessary external supportive devices or mobility equipment. Staff Training & Education: Provide regular training for direct care staff on safe transfers, gait exercises, and proper implementation of physical therapy techniques. Participate in the orientation and ongoing education of new staff, student interns, and volunteers. Collaboration & Consultation: Work closely with direct care staff, PTAs, aides, and other interdisciplinary team members to review treatment plans, monitor consumer progress, and adjust interventions as needed. Team Participation: Actively contribute to team meetings, clinical reviews, in-services, and quality improvement initiatives. Participate in goal development, coordination of services, discharge planning, and family education as appropriate. Equipment Oversight: Monitor and maintain physical therapy department equipment, ensuring proper functioning, repair, or replacement as necessary. Implement equipment effectively according to consumer goals and needs. Scheduling & Reporting: Schedule clients weekly as prescribed, reporting any changes in status, frequency, or progress to the supervising therapist. Communicate clearly with team members and other stakeholders regarding consumer progress, functional status, recommendations, and needs. Work Schedule On-call with flexible hours Operating Hours: Monday–Friday, 8 AM–5 PM; no weekends or holidays Hours vary based on needs and census levels Qualifications Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program Licensed Physical Therapist Member of the American Physical Therapy Association 2+ years’ experience with Adult Neurologically Disabled/TBI population preferred Why Join Our Team?
