We’re seeking a board-certified Geriatrician to step into a high-impact faculty and leadership role, advancing population health, age-friendly care, and whole-person health services for older adults – especially those living with dementia, complex conditions, and social vulnerability. This is more than a faculty position. It’s an opportunity to build, lead, and teach a better model of aging care. Why This Role Matters You will help shape a system where care is not fragmented–but integrated, compassionate, and aligned with what matters most to patients and their families. As a clinical leader, you’ll drive the development of an Age-Friendly Health System, embedding the nationally recognized Institute for Healthcare Improvement (IHI) 4Ms Framework into everyday care: What Matters – Care guided by patient goals and values Medication – Thoughtful prescribing that minimizes harm Mentation – Proactive dementia, delirium, and depression care Mobility – Preserving independence and function You’ll also help design longitudinal, whole-person care models that support not just patients – but the caregivers and families who care for them.
