Housing Case Manager

Job Summary: We are seeking a dedicated and compassionate Housing Case Manager to support individuals living with HIV in securing and maintaining stable housing. This role also involves working as a Medication Assisted Treatment (MAT) Patient Navigator to assist clients in accessing and adhering to their treatment plans. The ideal candidate will be skilled in both housing support and patient navigation within a healthcare setting. Although this role is unique in scope, it is a great opportunity for someone interested in case management and community outreach. Key Responsibilities: Housing Case Management: Assist clients in identifying and obtaining safe and affordable housing options. Conduct needs assessments to determine clients' housing requirements and develop individualized housing plans. Advocate on behalf of clients with landlords, property managers, and community organizations. Provide ongoing support to clients to ensure housing stability, including addressing any issues that arise with their housing situations. Facilitate access to community resources and support services to enhance clients’ overall quality of life. Medication Assisted Treatment (MAT) Patient Navigation: Assist clients in navigating the healthcare system to access MAT services. Provide education to clients about MAT, including its benefits, potential side effects, and adherence strategies. Collaborate with healthcare providers to coordinate care and ensure clients receive appropriate treatment. Monitor and support clients’ adherence to MAT protocols, including helping to address barriers to medication adherence. Facilitate communication between clients and healthcare professionals to ensure continuity of care. Documentation and Reporting: Maintain accurate and confidential client records, including housing and treatment plans, progress notes, and outcomes. Prepare reports on client progress and service outcomes for internal and external stakeholders as required. Track and manage caseloads effectively to meet established goals and deadlines. Community Outreach and Collaboration: Develop and maintain partnerships with local housing agencies, healthcare providers, and community organizations. Participate in community meetings and professional development opportunities to stay informed about best practices and emerging trends in housing and MAT services. Advocate for clients within the community and work to raise awareness about the needs and challenges faced by individuals living with HIV. Qualifications: Bachelor’s degree in social work, human services, public health, or a related field; or equivalent work experience. Experience in case management, housing support, or related social services, preferably with a focus on HIV/AIDS or chronic health conditions. Knowledge of Medication Assisted Treatment and experience working with clients undergoing MAT. Strong interpersonal and communication skills, with the ability to build rapport and effectively support clients from diverse backgrounds. Excellent organizational and time management skills, with the ability to manage multiple priorities and maintain detailed records. Proficiency in Microsoft Office Suite and experience with case management software is preferred. Ability to work independently and as part of a team in a dynamic and fast-paced environment.
