OTA or PTA Licensed Care Specialist

A company is looking for a Care Specialist to support members in improving their health through a Whole Person Care approach. Key Responsibilities Develop trusting relationships with members to facilitate behavior change and health management Assist in coordinating care and provide educational resources tailored to members' needs Create individualized health plans and serve as a resource for Care Management and team members Required Qualifications College degree in a health-related field preferred Current and unrestricted OTA or PTA license/certification required Minimum 1-3 years of clinical experience with patient assessments Experience with Motivational Interviewing techniques and working with the Medicare population Proficient computer skills and ability to work remotely
