This commitment applies to all aspects of the Health Federation of Philadelphia's employment practices, including recruiting, hiring, training, and promotion.JOB SUMMARYIntegrated Peer Models for Health Program (IPMH) is an established program of the Health Federation of Philadelphia that's funded by the Pennsylvania Department of Human Services to utilize and support Community Health Workers (CHWs) in serving populations facing multiple barriers to health and wellbeing, including behavioral health challenges, social drivers of health, and chronic disease, within the North Philadelphia Health Enterprise Zone (HEZ).The Program Coordinator will work with the IPMH Program Director and partners on the next iteration of this grant and will play an important role in supporting all aspects of the program's administration and implementation. Program activities will include leading work with partner organizations on the utilization of Connect4health, a resource and referral platform, coordinating professional development activities for our CHW and Supervisor learning collaboratives, and managing the roll out of a new CHW direct-service initiative, hiring CHWs to support behavioral health needs within partner organizations under the direction of the Program Director.To be successful, this individual must be: a reliable, self-starter with strong project coordination, organization, administration, and communication skills, who is collaborative, and adept at learning new processes and technologies.This position is full-time and supported by grant funding for the next 2-years.JOB SPECIFICATIONSResponsibilities/DutiesImplement project activities in collaboration with Project Director and partner organizations, including CHW hiring, and onboarding processesServe as a trusted touchpoint for project CHWs by providing regular check-ins to support the successful integration of CHWs into partner activitiesCoordinate and participate in engaging and robust trainings, in collaboration with the Program Director, colleagues, partner organizations, learning collaborative members, and trainersOversee all learning collaborative session administration, including registration and follow up: Create and send out invitations/registration forms, evaluations, and participation certificates, as well as timely emails to participants and consultants ahead of and following learning collaborative sessions.Lead Connect4health trainings with Connect4health partners and community organizationsIncrease the visibility and effective use of the findhelp/Connect4health platform across local community-based organizations focused on the North Philadelphia Health Enterprise ZoneLead Connect4health platform engagement and utilization activities with primary partner organizations, including the creation and distribution of a Connect4health partner newsletterContribute to program planning, monitoring, evaluation, reporting and improvement activitiesRepresent the Health Federation of Philadelphia in internal and external meetings/calls and regional collaborativesDocument minutes and manage partner follow up communicationsEducation: Master's degree in Social Work, Public Health or other relevant study areas preferred, or Undergraduate degree with required skills and experience.Skills/Experience3+ years of experience doing project coordination or administrative workExperience working with or as a Community Health Worker or Peer Specialist in a clinical or community-based organization, or deep knowledge of Community Health Work and/or behavioral health strongly preferredSome supervisory or 'lead' experience preferredOutstanding interpersonal skills, with a background in interacting with diverse people and organizationsComfortable working in a fast-paced environmentExcellent written and verbal communication skillsThorough attention to detail and organization skills are criticalDemonstrated ability to quickly and flexibly learn new processes and technologiesComfortable using Microsoft Suite, Google (e-mail, calendar, Drive), and other platforms, as neededDemonstrated ability to work independentlyWork Environment: The Program Coordinator will work in a 60/40 hybrid format (60% of the time in-office and 40% of the time virtual) and will report to the Health Federation of Philadelphia, 123 S. Broad Street, Suite 650, Philadelphia, PA 19109. Various meetings and training will be scheduled off-site throughout the city of Philadelphia.Position Type and Work Schedule: Full Time, 37.5 hours per week. Typical hours are Monday through Friday 8:30 am to 4:30 pm, but candidate must be willing to work a flexible schedule, including periodic evenings and weekends, as needed, with notice. Flex office schedule options available with supervisor approval.Continued funding for this project is not guaranteed.Travel: Some local travel to meetings and events at various sites may be required each month.
