Memory Care Coordinator

Strong interpersonal, leadership, and communication skills. Ability to plan, organize, and lead programs that are resident-centered and meaningful. Knowledge of dementia care approaches (e.g., validation therapy, Montessori for Aging, etc.) preferred. Certification as a CNA or Activity Professional (ADC, ADCP) is a plus, but not required. The Memory Care Coordinator oversees daily programming and care delivery within our Memory Care unit. You will be responsible for developing person-centered activities, fostering positive relationships, training staff, and ensuring a safe and supportive environment for residents living with Alzheimer’s disease or dementia. Responsibilities include: Plan, coordinate, and implement memory-care focused programs and activities that promote engagement, independence, and purpose. Support residents with ADLs as needed and help maintain a warm, therapeutic atmosphere. Collaborate with nursing, social services, dietary, and therapy teams to support coordinated care. Maintain care plans, activity documentation, attendance charts, and required regulatory records. Provide education and guidance to staff on dementia care best practices. Build positive relationships with families and serve as a compassionate liaison. Participate in care plan meetings, assessments, and quality assurance initiatives..
