Position Description The Recovery from Homelessness Program Interim Housing (IH) Case Manager will assess and triage all intakes and referrals for the Recovery from Homelessness Program (RFHP). The RFHP is a program that provides short-term housing opportunities for single adults in a 45-bed bridge shelter located in the City of Whittier. The program also offers housing navigation services to transition participants successfully from homelessness to long-term sufficiency. As an IH Case Manager, the individual will be responsible for reviewing and evaluating all applications to the program and determining eligibility and intake. This position requires a strong background in providing care to low-income populations, including those experiencing homelessness. The ideal candidate for this role will also be committed to providing quality programs and services and working well with a diverse and multi-disciplinary team. Specific Skills Required/Desired Program Responsibilities · Utilizing a harm reduction/housing first model, maintain a safe and supportive environment for all participants of the RFHP; help program participants adjust to the change and challenges in their lives including homelessness, unemployment, abuse recovery and family issues. · Review, evaluate, and verify all client intake information including but not limited to household size, income, criminal history, and other relate information to assure compliance with program eligibility guidelines. · Obtain and input all required client information into the Homeless Management Information System (HMIS) database working closely with the CES Program Director to minimize errors and duplicate entries. · Log, file and enter source documents and related data into client files, HMIS, and other data capture systems. · Generate eligibility and ineligibility letters to applicants as well as participant progress reports and letters. · Respond to email, phone and mail inquiries for the RFHP. · Liaison with probation officers, law enforcement, substance use counselors, foster agencies, and other community groups for intake of potential program participants. · Coordinate and/or conduct group and individual intake meetings with new program participants to explain shelter policies and procedures. · Maintain, manage and track waitlist of potential participants for all bridge housing programs. · Process changes and updates to applicants’ records to re-determine eligibility for programs as applicants’ status changes. · Prepare regular summary reports of waitlist, bed count figures and program rosters. Generate a variety of technical, statistical and administrative reports to assist management in compiling information necessary to comply with county, state and federal requirements and audits. · Maintain client related data tracking systems, including case notes program rosters and HMIS entries. · Assist with management of client progress through the RFHP by coordinating with Housing Navigators, Shelter Manager, and Transitional Case Manager to ensure that program participants have undergone orientation, are completing their Passports to Success and are progressing on fulfilling their individualized care plans. · Participate in program, department and agency meetings and trainings as required, as well as inter-agency groups that provide assistance to individuals experiencing homelessness. · Maintain and execute confidential information according to HIPAA standards. · Attend staff trainings and department meetings as needed and other duties as assigned.
