Promotes person-centered care for optimal quality of life. Collects and interprets data. RN's record resident/patient vital signs and information about their condition. RN will coordinate with physicians and other healthcare team members to ensure quality and appropriate care is provided. RN may monitor physical activity and nutrition. RN may oversee the work and provide support to LPN's, CNA's & Aides.Essential Job RequirementsEducation: Two (2) year Associates DegreeLicense: Active New York State RN licenseQualifications: BLS and/or CPR certification (Highly preferred)Must be 18 years of age. Ability to read, write and speak the English languageExperience: Prior RN experience preferredEssential Job Duties & ResponsibilitiesAuthority delegated to;Provide treatment to residents in accordance with physician ordersAdminister medications to residents in accordance with physician ordersAssess resident needs & take appropriate actionEmergency care to residentsSupervise CNA's & LPN'sSupervision of performance and conductProvide clinical supervision, evaluation and action as required to LPN, CNA and AidesAssist in evaluating performance, coaching & discipline of LPN, CNA & AidesProvide job training and coachingAdministering & documentation of treatmentsObtain physician orders and any revisions for resident treatmentsAdminister treatments and accurately record treatmentsFollow comprehensive care plans related to treatment needs and assessmentAdministering & documentation of medicationAdminister medication to residents in an accurate and timely mannerMonitor effectiveness & adverse effectsComplete all RX documentationOrder, receive & store medications appropriately and per policiesMaintain security of med cart and roomCommunication of resident & staff informationObtain report from nurse being relieved by shift change and document actions related to treatment or medicationProvide coming on duty nurse with concise, accurate reportReport changes in resident's condition, to include house charge and physicianInterdisciplinary Care planningDocument resident care provided and resident response (or lack of)Review & update care plansParticipate in interdisciplinary team and rehabilitation meetingsConduct resident rounds.
