Direct Support Professional (DSP) for Brooklyn

Program: Day Habilitation Without Walls · $250 Sign-on bonus. Exemption Status: Non-Exempt Reports To: Assistant Director of Day Habilitation Services General Summary: Provides direct support and care for adults with developmental disabilities. Facilitates attainment of person-centered goals for program participants. Responsible for documenting the provision of supports. Major Job Duties and Responsibilities: · Drives City Access New York vehicle to transport participants to and from homes to community-based program settings. · Chaperones participants into the community on a daily basis and maintains close supervision of them. · Collects data for written program plans, i.e. daily notes, behavior plans, treatment plans. · Writes case notes and provides information on incidents as required. · Provide necessary program support/coverage. · Participates in community-based activities, to provide support and supervision of participants. · Provide physical and programmatic aid to participants where assigned. · Aids in daily operation of assigned program areas. · Assists with supervision of participants in assigned areas. · Initiates precautions within established framework to assure that work is completed in a manner free from recognized hazards. · Notifies supervisor of any problems with the vehicle. · Participates in case conferences as needed. · Serves as member of interdisciplinary team. · Attends periodic supervisory conferences and required staff meetings. · Participates in in-service training programs. · Substitutes in other groups as needed. · Performs pre-trip inspection of vehicle in accordance with City Access New York policy. · Maintains vehicle log of mileage time and ensures that it is kept clean. · Performs other related duties as requested. Minimum Education and Experience Requirements: High School Diploma or GED. At least one year of comparable driving experience. Six months of related direct care experience preferred. License/Certification: Valid New York State Driver’s License, maintained in good standing. Completion of Drivers Safety Course is mandatory upon hire. Skills and Abilities Required: · Good oral and written communication skills. · Good interpersonal skills necessary to work effectively with coworkers, employees, participants, families and outside contacts. · Ability to anticipate, analyze, and develop responses to program logistics and interpersonal relationship problems · Must be able to maintain the strictest confidentiality. Physical Demands: · Ability to maintain focus and safety while driving. · Ability to provide reasonable assistance to participants’ physical needs. · Stooping, bending, kneeling and pushing and pulling of heavy objects. Working Conditions: · As needed, providing personal care assistance to participants. · May be transferred to another program site to meet CANY needs. · Occasional extension of shift to cover the health and safety needs of participants as emergencies and/or other situations warrant. · May at times be required to work overtime. · May need to take special precautions in the handling of body fluids.
