Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well-being of our residents.Work EnvironmentWork is performed in a skilled nursing facility with potential exposure to infectious diseases and hazardous materials. Appropriate personal protective equipment (PPE) is provided.ResponsibilitiesPatient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.Monitoring and Assessment: Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care. QualificationsLicensure:Valid LPN license in OKEducation: Graduation from an accredited LPN program.Experience:Previous experience in a skilled nursing facility or similar healthcare setting preferred.Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.Communications:Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.Physical Requirements:Ability to lift and move patients, perform physical assessments, and stand for extended periods.
