In this role, you will evaluate patients, develop individualized care plans, and provide therapeutic interventions in the comfort of patients' homes. You will play a critical role in improving patient mobility, function, and independence while delivering personalized, high-quality careKey ResponsibilitiesPerform comprehensive patient evaluations and develop individualized treatment plansProvide skilled physical therapy services in accordance with physician ordersRegularly reassess patient progress and modify treatment plans as neededSupervise and collaborate with Physical Therapy Assistants (PTAs)Educate patients and caregivers on treatment plans, home exercise programs, and safety strategiesComplete accurate and timely documentation in compliance with regulatory standardsCoordinate care with interdisciplinary team members, including nurses, physicians, and other therapistsTravel to patient homes within assigned service area QualificationsDoctor of Physical Therapy (DPT) or equivalent degree from an accredited programCurrent Physical Therapist license in FLMinimum of [1 year preferred] experience (home health experience preferred)Current CPR certificationValid driver's license, reliable transportation, and auto insuranceStrong clinical decision-making and problem-solving skillsExcellent communication and organizational abilitiesAbility to work independently and manage a flexible schedule Why Join Us?We are committed to delivering patient-centered care that promotes independence and enhances quality of life.
