RN Case Manager - HSNNY

Health Services of Northern New York is seeking a Registered Nurse Case Manager in St. Lawrence County. This position will be responsible for the management of client care in the home setting. The Registered Nurse Case Manager will perform nursing assessments, coordinate care with physicians, and have oversite of additional discipline involvement with each client. Additional discipline involvement could include Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker and Home Health Aides. This position will require travel to and from patient homes throughout St. Lawrence County. This is a full-time position Monday – Friday as well as some rotating on-call requirements. An ideal candidate will have excellent communication skills and be comfortable traveling to and working one-on-one with patients in their homes. Additionally, the candidate will be flexible, enthusiastic and passionate about patient care. Minimum starting salary for this position is $68,500.
