This field-based role focuses on delivering individualized care, managing complex clinical needs, and supporting patients in a home setting to improve health outcomes and quality of life.Schedule & Work Details:Location: Howard and Carroll County coverage areaSchedule: Monday through Friday, 8:00 AM to 4:30 PMWeekly Guarantee: 32 hoursOn-Call/Weekend Requirement: On-call requiredMileage Reimbursement: Yes, between patient visits (first to last patient)Pay Rate: $51 to $53 per hourDress Code: Navy blue scrubsKey Responsibilities:Provide skilled nursing care to patients in their homes in accordance with physician orders and plan of carePerform patient assessments, identify changes in condition, and adjust care plans as neededAdminister medications, treatments, and specialized proceduresManage complex clinical tasks including central line care, wound care, wound vacs, and catheter care (foley and suprapubic)Perform venipuncture and specimen collectionDocument patient care accurately and timely in accordance with agency and regulatory standardsCollaborate with physicians and interdisciplinary care team to coordinate patient careEducate patients and caregivers on disease management, medications, and home care techniquesRequired Qualifications:Active Registered Nurse (RN) license in the appropriate statePrior home health experience requiredStrong clinical assessment and documentation skillsAbility to work independently and manage a field-based schedulePreferred Qualifications:Experience with pleurex drainsExperience with central line careProficiency in wound care and wound vac managementExperience with catheter insertion (foley and suprapubic)Strong venipuncture skillsExcellent time management and communication skillsExperience with Homecare Homebase (HCHB) is a plusPride-Health offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.Equal Employment OpportunityPride-Health is an equal opportunity employer.
