This position functions as alead coordinator in a rural setting with limited resources, focusing onprogram implementation, cross-system collaboration, and strengtheningaccess to care. The position works closely with Overdose Fatality Review(OFR) team members, including the nurse and peer support staff, toimprove community response and reduce overdose risk**Essential Job Functions**(Essential function, as defined under the Americans with DisabilitiesAct, may include the following tasks, knowledge, skills and othercharacteristics. This list of tasks is ILLUSTRATIVE ONLY, and is not acomprehensive listing of all functions and tasks performed by positionsin this class.)*\\Coordinates substance use disorder (SUD) prevention, risk reduction, andrecovery support programs and activities across the County.\Works collaboratively with the Overdose Fatality Review (OFR) team,including the OFR nurse and peer support staff, to identify trends,service gaps, and opportunities for intervention.\Supports development and implementation of care coordination andreferral systems in a rural environment with limited existinginfrastructure.\Completes records and reports; ensures timely submission of requiredreports to state and funding agencies; reviews program goals andprogress toward outcomes.\Meets with coalitions, healthcare providers, behavioral health agencies,law enforcement, and community members to assess needs, strengthenpartnerships, and coordinate limited local and regional resources.\Assists in developing and maintaining a network of service providers,including out-of-county and regional partners, to address gaps intreatment, recovery, and support services.\Reviews program budgets and assists with financial tracking, grantimplementation, and reporting requirements.\Prepares reports regarding community health concerns, includingsubstance use trends, overdose data, and program outcomes, andcontributes to development of planned courses of action.\Coordinates with staff of other health programs within the healthdistrict to ensure integrated service delivery for mutual clients andshared public health priorities.\Provides training and technical assistance to staff and communitypartners on topics such as risk reduction strategies, overdoseprevention, and resource navigation.\Oversees tracking of program activities, referrals, services provided,and outcomes to ensure quality services and compliance with grantrequirements.\Participates in and represents the department at local, regional, andstate meetings related to public health and substance use initiatives tostay current on best practices and funding opportunities.\Travels between County sites and regional locations for meetings and tosupport program coordination efforts.**Knowledge and Skills** Knowledge of the principles, practices and trends of public healthadministration, particularly related to substance use disorders andrural health systems Knowledge of laws, codes, ordinances, and standards pertaining topublic health, behavioral health, and preventative services. Knowledge of principles of grant writing, grants administration, andspecialized grant fund reporting requirements and procedures. Knowledge of care coordination, referral systems, and servicedelivery in resource-limited settings. Skill in analyzing the needs of the department and the County;developing short- and long-range plans to address gaps in services. Skill in effectively communicating verbally and in writing. Skill in establishing and maintaining effective workingrelationships with employees, community partners, other agencies,and the public. Skill in planning, organizing, and coordinating complex programactivities across multiple systems. Skill in developing partnerships and leveraging limited resources tomaximize community impact.**Desirable Qualifications**Bachelors Degree in Public Health Administration, Social Work,Behavioral Health, or related field; AND three years experience workingin a health field; AND a valid drivers license; OR equivalentcombination of education, training and experience. Experience insubstance use disorder programs, rural health, program coordination, andgrant-funded initiatives is strongly preferred.
