Internal Medicine - Geriatrics Physician

- Make an impact on seeing the underserved population- Seeking a Board-Certified Geriatric Medicine Physician- Bi-lingual English / Spanish- Outpatient responsibilities only- Full-time permanent position- Monday " Friday- No nights, no weekends- Small patient panels for consistency and focus- 12-15 patients per day-Value-based care model and competitive bonuses are structured to reward outcomes-Provide personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest- Location: Elgin, IL- 35 miles NW of Chicago along the Fox River.
