Role Description This is a temporary, fully remote role for a Health Care Coach. The Health Care Coach will work closely with patients to provide guidance and support in achieving their health and wellness goals. Responsibilities include creating personalized care plans, conducting health assessments, offering actionable advice, and liaising with healthcare providers, as needed. The Health Care Coach will play a critical role in empowering patients to manage their health effectively and improve outcomes. Qualifications Strong communication and interpersonal skills, with the ability to articulate medical and wellness information in a clear, empathetic mannerKnowledge of health coaching, patient engagement, and care-management practicesProficiency in health education, establishing patient goals, and behavior modification techniquesAbility to adapt to remote work environments, demonstrating independence, time management, and organizational skillsExperience in healthcare, nursing, social work, or a related fieldRelevant certifications in health coaching, nursing, or other related areas are preferredFamiliarity with electronic health record (EHR) systems and digital tools is a plusCommitment to confidentiality and adherence to healthcare compliance and regulations.
