Job Summary: The Registered Nurse is responsible for the delivery of quality patient care services.Responsible for establishing a Plan of Treatment individualized for each patient and supervise the staff delivering the care.You will build relationships with patients and families as you visit them within the comfort of their own homes.Conduct Comprehensive Assessments (using the OASIS tool), develop and implement Plan of TreatmentCollaborate with physicians, agency staff and contract services to coordinate patient careInitiate appropriate referrals for other medical services when indicatedPerform various types of skilled nursing visitsPromote and maintain patient health through teaching and learning appropriate self-care techniquesDocument direct patient services in a timely manner in electronic medical recordsCollaborate with management team and office staff to facilitate timely office processing of paperwork and patient informationParticipate in agency quality improvement activitiesMaintain the highest level of ethical standardsQualifications:Must be a Registered Nurse (RN) with the Illinois Department of Professional RegulationMust have a minimum two years of home health experiencePsychiatric Nursing experience a major advantageCurrent CPR certificationMust have and maintain a valid IL driver's license, automobile insurance coverageMust be willing to travel to surrounding areas and patients' homesAbility to work independently with minimal supervision and time management abilitiesAbility to successfully complete required background checkExcellent communication and reporting skillsHome Health: 2 years (Required)Benefits:401(k)Dental insuranceHealth insuranceLife insurancePaid time offReferral program
