Now Hiring: Care Coordinator (Remote | Medicaid Programs)We're looking for a detail-oriented and compassionate Care Coordinator to support members through outreach, enrollment, and ongoing care management. This role is ideal for someone who thrives in a fast-paced, team-driven environment and is passionate about improving access to healthcare and social services.Key ResponsibilitiesMember Outreach & EnrollmentConduct outbound outreach to assess member needs and determine eligibility for support programsGuide individuals through enrollment in Medicaid 1115 Waiver programs available at no costCollect and document key information to connect members with appropriate clinical and community resourcesCare Coordination & EngagementManage referrals, care transitions, and care plans to ensure timely and effective service deliveryPerform proactive outreach via phone, text, and digital platforms to support care plan adherenceIdentify and help resolve barriers to care, including transportation, financial limitations, and access challengesCoordinate services across healthcare providers, specialists, and community-based organizationsEducate members and caregivers on care plans, follow-ups, and available resourcesSupport members in navigating healthcare systems and community servicesApply motivational interviewing and culturally responsive communication techniques to drive engagementReferral Management & Quality OversightTrack referral progress and ensure completion with proper documentation and follow-upCommunicate with providers and partners to confirm appointments, outcomes, and next stepsEscalate delays or gaps in care according to established protocolsMaintain accurate and timely documentation within care management systemsEnsure compliance with HIPAA, regulatory requirements, and internal policiesMonitor performance metrics such as referral completion rates and care gap closuresCollaborate with interdisciplinary teams to deliver high-quality, coordinated careParticipate in team meetings and care reviews to support continuous improvementRequired QualificationsPrior experience in care managementReliable high-speed internet connection with Ethernet capabilityAbility to work in a quiet, distraction-free environmentStrong teamwork mindset and adaptability in a changing environmentPreferred QualificationsBilingual skillsExperience in remote care managementScheduleMonday–Friday, 9:00 AM – 5:30 PM ESTFlexibility to work occasional Saturdays as needed
