Intake Referral Specialist

Family Home Health and Hospice - The Family Home Health Network, a partner to the Alden Network, is a dynamic post-acute care service provider of intermittent home health care, palliative care, hospice care and skilled/custodial private care services throughout the Chicagoland area.We are looking for an Intake/Operations Coordinator experienced with home health to add to our team who will be responsible for various aspects of clinical operations including gathering/data entry of referrals, admissions, discharges, scheduling of staffing needs, customer service calls/follow-up, administrative coordination of services and other related clerical functions supporting administrative processes. Ideal candidates will have experience with home health, hospice, private duty, physician services or similar healthcare services.THIS POSITION IS MONDAY-FRIDAY 8:30AM TO 5:00PM****THIS ROLE DOES REQUIRE WEEKEND ON CALL EVERY 5-6 WEEKS***Applicants MUST have:Minimum of 1 year of intake experience is required.High computer literacy/capabilities required (proficiency with Microsoft Word and Excel, and Electronic Faxing.Excellent verbal and written communication skillsOutstanding customer service attitudeDemonstrated knowledge of insurance benefits/plans and medical terminologyPreferred:Intake home health or hospice experienceHCHB EMR experience is preferredAs a member of our team, you will enjoy a competitive salary and generous benefit package as well as being a part of a dynamic team-oriented environment.Family Home Health Network is committed to attracting and attaining the best talent.
