Through consistent, relationship‑based case management, you help clients access housing, strengthen life skills, and remain stably housed in the community.This role is ideal for someone who believes housing is a human right, values trauma‑informed and harm‑reduction approaches, and wants to do meaningful work in a mission‑driven organization.Avivo is a 32‑hour work week organization, offering full‑time pay and benefits while supporting balance, sustainability, and wellbeing.What You’ll DoBuild trusting, person‑centered relationships with individuals and families experiencing homelessnessMaintain an assigned caseload and meet with clients regularly through in‑person and creative outreachComplete strength‑based assessments and partner with clients to create housing‑focused goal plansSupport clients while they are in shelter or temporary settings to prepare for housing opportunitiesAssist clients in locating, obtaining, and maintaining scattered‑site housingCoordinate services related to medical care, mental health, substance use, income, and benefitsSupport housing stability through landlord communication, eviction prevention, and mediation when neededHelp clients understand tenant rights and responsibilitiesDocument services and housing activities accurately and on time in required systemsCollaborate closely with teammates, supervisors, and community partnersPractice Avivo’s person‑centered, trauma‑informed, harm‑reduction, and anti‑oppression approachesQualificationsWhat You Bring High school diploma or GEDOne year of experience working with adults experiencing homelessness or similar populationsOR an associate degree or higher (no experience required)Lived experience may be considered as part of the experience requirementStrong relationship‑building and communication skillsAbility to work with individuals facing complex barriers with compassion and accountabilityInterest in housing‑first, trauma‑informed, and harm‑reduction practicesBasic comfort with documentation and learning multiple data systemsReliable transportation, a valid driver’s license, and ability to travel within the communityFlexibility to meet client needs, including occasional evenings or on‑call coverageWhat We Offer32‑hour full‑time work weekMedical Health PlanHealth Savings Account (HSA)Nice Healthcare — chat, video visits, and in‑home primary careDental PlanDependent Care Flexible Spending Account (FSA)Basic Life/AD&D & Voluntary Life/AD&DLong‑Term DisabilityOption to purchase critical illness, accident insurance, and short‑term disability401(k) Plan10 paid holidays & generous PTOHow To ApplySubmit your application online through Avivo’s Careers page at Avivomn.org.
