Home Health Aide
Reports to: Clinical Director Classification: Part-time/PRN; non-exemptSetting: Patient homes and home officeSummary: Home Health Aides work closely with patients, their families, and healthcare professionals to promote overall well-being and improve the quality of life for individuals who are elderly, disabled, or recovering from illness or surgery.Key Responsibilities:Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and eating.Help with mobility, including transferring clients from bed to wheelchair or assisting with walking.Provide companionship and engage in activities to support mental and emotional well-being.Assist with light housekeeping, meal preparation, and laundry as needed.Monitor and report any changes in the client’s physical or emotional condition to the appropriate supervisor or healthcare team.Help with medication reminders, ensuring medications are taken as prescribed.Follow care plans developed by healthcare professionals to ensure clients' needs are met.Maintain accurate documentation of services provided and any changes in client conditions.Monitor vital signs and appropriately report signs/symptoms of concern or illness to Care Manager or Executive Director of Clinical ServicesIf patient has gone to hospital or doctor for care/concerns, report changes to medicationQualifications:High school diploma or equivalent.Certification as a Home Health AideCompassionate, patient, and able to build trusting relationshipsAbility to perform light housekeeping and other daily living tasksAbility to maintain confidentiality and adhere to privacy standards (HIPAA)Reliable, punctual, and able to follow directions with minimal supervisionMust have reliable transportation to travel to clients’ homes.Physical Requirements:Ability to lift up to 25 pounds.Ability to stand, sit, bend, and kneel for extended periods.Ability to assist clients with mobility and transfers.While performing the duties of this job, the employee is regularly exposed to an office environment in which the noise level is usually low to moderate.
